STATEMENT OF NEED

Head and neck cancer represents a heterogeneous group of tumors that originate in the oral cavity, pharynx, larynx, and paranasal sinuses. An estimated 59,660 new cases will be diagnosed in the United States in 2025, and 12,770 people will die of the disease (ACS, 2025). Although only approximately 10% of patients have distant metastasis at diagnosis, a substantial proportion develops metastases following first-line treatment for locoregional disease (Pisani et al, 2020). Despite advances and innovations in multimodality treatment and improved understanding of head and neck carcinogenesis, the prognosis of head and neck squamous cell carcinoma (HNSCC) remains poor, with a 5-year relative survival rate of 69% from 2014 to 2020 and median overall survival of approximately 1 year in the recurrent/metastatic setting (ACS, 2025; Haring et al, 2023). Given the increasing use of immune checkpoint inhibitors in the treatment of recurrent and metastatic HNSCC, understanding guideline recommendations for treatment selection, follow-up, and management of immune-related adverse events (irAEs) is critical for optimizing outcomes. Advances in treatment have improved survival rates, necessitating a comprehensive focus on supportive care and survivorship planning to enhance quality of life and long-term outcomes. This educational guide will provide key insights into the evolving role of immune checkpoint inhibitors for metastatic HNSCC and the multidisciplinary approaches essential for effective care and follow-up.

TARGET AUDIENCE

Medical oncologists, advanced practice providers, pharmacists, nurses, and other healthcare professionals involved in the care of patients with head and neck squamous cell carcinoma (HNSCC).

LEARNING OBJECTIVES

Upon completion of this activity, participants should be able to

• Apply evidence-based criteria for selecting immune checkpoint inhibitor therapy in metastatic head and neck squamous cell carcinoma (HNSCC)
• Implement appropriate monitoring and management strategies for immune-related adverse events, particularly hypothyroidism
